Dreams2Live4 has been launched as an Executive Fundraising Committee of the Prince of Wales Hospital Foundation, and therefore, at this stage, the fulfilment of dreams requiring funding can be extended only to patients of the Prince of Wales Hospital.

At a later stage, it is hoped that the work of Dreams2Live4 will be expanded to include all metastatic cancer patient sufferers, and dream applications will be considered and assisted where services are available, or have already been offered.

The procedure to apply for a Dream to be fulfilled is as follows:

• Please complete and submit the Dreams2Live4 Dream Application Form

• Please print a copy of this form.

• Completed forms will be reviewed by the Dreams2Live4 Committee as to whether they are viable, and can be assisted.
• Confirmation is required from the applicants Doctor, or approved medical practitioner as to the patients’ suitability to have their dream fulfilled.
• Once approved, the committee will appoint a designated coordinator to communicate with the applicant and the parties involved.
• Contact will be made with the applicant to determine specific requirements that the dream will require, who will report back to the committee.
• The applicant will be required to sign related documents and legal indemnities relating to the category of their dream.
• The Committee will endeavour to source and confirm the services or items required for the fulfilment of the dream.
• If the dream can be sourced and provided, the committee will co-ordinate associated PR and media coverage.
